当科における原発不明頸部癌の臨床的検討

Cervical Lymph Node Metastasis from Unknown Primary Sites

　はじめに

　頭頸部領域では，頸部転移巣のみで原発巣が確認できない，いわゆる原発不明癌は全頭頸部癌の1〜5％を占めており^1），初診時すでにN1，Stage III以上の臨床病期であることからその予後は不良で，5年生存率はほぼ20〜50％とされている^2,3）。全腫瘍に占める割合は低いが，診断技術の進歩にもかかわらずその割合は減少傾向は示しておらず^4），絶えず臨床的な問題を提起している。

　今回，当科において経験した原発不明頸部癌の症例について，診断に至る経過，そして原発巣の処理と頸部転移巣の対処の仕方を検討し，臨床的な問題点をあげて考察した。

We reviewed 9 patients treated with surgery and/or radiotherapy or with combined chemotherapy. There were 6 patients with squamous cell car-cinoma, 2 with undifferentiated carcinoma and one with mucoepidermoid carcinoma.

All the patients achieved complete response to primary treatment. Two of them recurred, one at neck and the other at the larynx.

Eight patients have been alive with no evidence of disease. One died with other disease.

The results suggested that cervical lymph node metastasis from unknown primary sites must be considered as a clinical entity. And the treatment should be based on comprehensive view.
