両側性Zenker憩室の1症例

A Case of Bilateral Zenker's Diverticula

　はじめに

　頸部食道憩室は，大部分が無症状で，X線造影で偶然に発見されることが多い。その成因については多くの要因が関連すると考えられてはいるが，嚥下時の輪状咽頭筋と上部食道の協調不全が深く関与するとされる。今回われわれは，嗄声を唯一の症状とした両側性頸部食道憩室を経験し，憩室切除および輪状咽頭筋切除により治療したので，若干の文献的考察を加えて報告する。

Esophageal diverticulum is asymptomatic in most cases and is rarely considered as the indication for surgery.

A 62-year-old male complained of hoarseness.On clinical examination, both vocal cords were atropic and reddish. Laryngeal radiography revealed two abnormal masses in cervical esopha-gus. Esophageal fluoroscopy revealed bilateral eso-phageal diverticula. The size was 45×25×40 mm at the right side, 10×7×6 mm at the left. The patient underwent diverticulectomy combined with crico-pharyngeal myotomy. No apparent recurrence was found for 3 years.
