当科における下咽頭癌の臨床統計—両側頸部郭清の適応を中心に

Statistical Analysis of Hypopharyngeal Carcinoma

　はじめに

　下咽頭癌は，頭頸部癌のなかでは予後不良の疾患の1つである。StageⅢ，Ⅳといった，いわゆる進行癌の状態で初診することが多く，手術，化学療法，放射線療法を併用した集学的治療を行っている施設が多い^1,2）。しかしその術式，特に頸部郭清術の適応に関しては，施設によりさまざまである^1〜3）。当科においては，基本的にはneo-adjuvant chemotherapy施行後に咽喉食摘，両側頸部リンパ節郭清を行い，症例によって術後照射を併用してきた。今回，国立東静病院耳鼻咽喉科にて初回治療を行った下咽頭癌症例をまとめるとともに，その術式について検討する。

Of 38 patients diagnosed with hypopharyngeal squamous cell carcinoma at our department between September 1988 and August 1996, 30 patients underwent surgical procedures including unilateral or bilateral neck dissection. Twenty five of the cases were advanced cases. The accumulative five-year survival rate was 66.75% in the cases treated with surgery. To study the indication of an elective neck dissection, TN classification was compared with pTN classification in these cases. The elective neck dissection should not be indicated in PS type cases which were evaluated as either T1 or T2 with NO, N1 or N2a.
