鼻副鼻腔平滑筋肉腫の1例

A Case of Leiomyosarcoma in the Ethmoid Sinus

　緒言

　鼻副鼻腔における平滑筋肉腫は非常に稀な疾患である。われわれは，64歳の男性で再発した鼻副鼻腔平滑筋肉腫に対し，冠状頭皮切開で前頭蓋底経由による腫瘍摘出術を行い良好な結果を得たので文献的考察を加え報告する。

We report a case of recurrent leiomyosarcoma in the ethmoid sinus in a 64-year-old male. CT and MRI revealed invasion to the anterior skull base. The tumor was resected by the transbasal approach. Histological examination revealed interlacing bun-dles and whorls of spindle-shaped cells with blunt-ended nuclei. Chemotherapy and radiation therapy are ineffective in leoimyosarcoma. Surgery is the treatment of first choice. Leiomyosarcoma in the ethmoid sinus with the invasion of the base of skull should be resected by the transbasal approach because complete resection contributes to a good outcome.
