緊急気管切開を要した扁桃周囲炎症例

A Case of Peritonsillar Cellulitis requiring Tracheotomy

　はじめに

　扁桃周囲炎は深頸部感染症のなかでは最も頻度が高く^1,2），日常診療のなかでしばしば遭遇する疾患であるが，大部分の症例は抗生物質投与と外科的治療（切開排膿）により比較的速やかに治癒し，重篤な合併症を生じることは稀である。しかし今回われわれは，入院中に突然の気道閉塞を生じ，気管切開を余儀なくされた症例を経験したので，若干の文献的考察を加えて報告する。

A 23-year-old man was admitted to our hospital with a five-days' history of sore throat and high fever. Physical examination of the oropharynx revealed inflamed tonsils and a tense swelling of the peritonsillar region. Aspiration of the peritonsillar space yielded no pus. The following day of admis-sion, he developed sudden severe dyspnea and was treated with emergency tracheotomy. A tra-cheotomy tube was decannulated on the sixth post-operative day and the patient was discharged on the twelfth hospital day.
