輪状甲状靱帯切開を要した緊急気管切開術の1施行例

A Case of Cricothyrotomy Necessitated Prior to Ordinary Tracheotomy

　はじめに

　輪状甲状靱帯切開は，異物，炎症，外傷などによる急性の上気道閉塞に対して，気管内挿管困難や，気管切開の時間的余裕がない場合の救急処置として行われる^1）。その手技については教科書的にも記載があり普遍的なものであるが，実際にこれを要する症例を経験することは比較的稀と考えられる。今回われわれは，輪状甲状靱帯切開を要した緊急気管切開術の施行例を経験したので，若干の文献的考察を加えて報告する。

Emergency tracheotomy was planned for a 63-year-old female complaining of dyspnea caused by acute epigottitis. Immediately after the induction of general anesthesia, her respiration was arrested and the upper airway obstruction prevented the use of the bag and mask technique.

Cricothyrotomy was immediately performed and the patient recovered successufully.
