気道確保を要した急性喉頭蓋炎の3症例

Three Cases of Acute Epiglottitis

　はじめに

　急性喉頭蓋炎はしばしば急激な経過をとって呼吸困難が悪化し，時には症状初発から数時間のうちに気道閉塞に陥ることもあるため，その治療にあたっては気道確保が最も重要な問題となる。特に小児においてはその緊急性が高い^1〜3）。

　一方本疾患は欧米では1〜5歳の幼小児に好発するといわれているが^1〜5），本邦における報告例^6〜9）は驚くほど少ない。

　われわれは1990年4月から1991年5月までの約1年間に，気道確保を要した急性喉頭蓋炎症例を3例（小児2例，成人1例）経験したので，若干の文献的考察を加えて報告する。

Three cases of acute epiglottitis were treated by artificial airway. In two cases of children, the problems on artificial airway required for acute epiglottitis were discussed.

Case 1: A 40-year-old male who complained of increasing sore throat and dyspnea was treated by emergency tracheostomy. Case 2: A 3-year-old male developed respiratory arrest 45 minutes after admission. Oral tracheal intubation follow-ed by tracheostomy was performed. Case 3: A 8-month-old female who suffered from dyspnea was treated by oral tracheal intubation. All patients recovered completely without any compli-cation.
