回転性めまいで発症した後下小脳動脈解離性動脈瘤症例

Dissecting Aneurysm of Posterior Inferior Cerebellar Artery:Report of a Case

　はじめに

　頭蓋内に発生する解離性動脈瘤については近年，脳神経外科領域や神経内科領域からの報告が増加している^1〜14）。しかし，その多くは主幹動脈に生じるもので，分枝動脈に生じることは極めてまれといわれている^2）。組織学的に解離性動脈瘤と確認できたManzら^5）の報告では，内頸動脈系では中大脳動脈起始部に多く，椎骨・脳底動脈系では椎骨動脈や脳底動脈に多い。

　今回われわれは，回転性めまいで発症し，典型的なワレンベルグ症候群の症状を呈した後下小脳動脈の解離性動脈瘤というまれな1例を経験したので報告する。

A 38-year-old man was complaining of rotary vertigo, hoarseness and occipital headache. Neur-ological examinations revealed these symptoms and signs compatible for Wallenberg's syndrome. Verte-bral angiograhy revealed a dissecting aneurysm on the region from the anterior medullary segment to the lateral medullary segment of the right posterior inferior cerebellar artery (PICA). MRI demonstrat-ed an infarction of the right dorso-lateral region of the medulla. Dissecting aneurysm of PICA is rare. The clinical features, pathogenesis, and treatment are discussed by reviewing literatures.
