反対側に伝音性難聴を伴った小脳橋角部腫瘍の1症例

A Case of Cerebellopontine Angle Tumor with Contralateral Conductive Hearing Impairment

　はじめに

　一側に高位頸静脈球を伴う癒着性中耳炎，反対側に聴力正常で耳鳴のみを症状とする小脳橋角部腫瘍を合併した1例を経験した。患者は癒着性中耳炎側の聴力改善を希望して受診したが，最終的には小脳橋角部腫瘍の手術を優先して行った。本例は術後，残念ながら聾となったが，伝音難聴側に補聴器を装用し，日常生活を行っている。本例の診断，手術に至る経過の問題について考察を加え，報告する。

A 53-year-old female presented with a hearing impairment of her right ear and tinnitus of her left ear. It was considered from the examinations to determine indication of tympanoplasty of her right ear that the high jugular bulb and otitis media caused her hearing disturbance. CT scan revealed a cerebellopontine angle tumor of her left ear. It was recommended to follow up the patient with tinnitus keeping of a risk of occurrence of tumor in mind.
