側頭骨鱗部に発生した巨大真珠腫の1例

A Case of Giant Cholesteatoma of the Temporal Squama

　はじめに

　今回われわれは，37年前に中耳根本術を受け，最近になって外耳道から水様性の耳漏を呈し，画像診断で側頭骨鱗部に巨大な腫瘤を形成している所見をもつ症例を経験した。手術所見では側頭骨鱗部の外板は紙のように薄くなり，内板は大きく小脳硬膜を圧排し，板層間には巨大な真珠腫が認められた。水様性耳漏は脳脊髄液瘻であった。中耳炎手術既往があるので，板間層の類上皮腫（＝先天性真珠腫）とは考えにくいが，成長がゆっくりしていること，その好発部位が頭蓋内では小脳橋角部，脳下垂体周囲であり，また板層間にも発生し，側頭骨や頭頂骨に好発することから^1），診断に苦慮した。この症例の発生機転，鑑別診断などにつき若干の文献的考察を交えて報告したい。

A 53-year-old woman presented with her right hearing loss and watery discharge for 3 years. She had radical mastoidectomy 37 years ago. X-ray studies demonstrated a lucent area in the right temporal squama behind the ear. CT revealed an expansive mass in the diploe. Surgical exploration demonstrated a 3×4.5 cm cholesteatoma. Patho-genesis and differential diagnosis of cholesteatoma were discussed.
