A 72-year-old man developed acute left facial paralysis with sudden deafness and dizziness. MRI examination revealed a small tumor localized in the internal auditory canal (IAC). His left hearing was completely lost and the facial nerve was severely damaged, then the tumor was removed by trans-labyrinthine approach and the facial nerve was decompressed from the IAC segment to the mastoid segment at two months after onset. Facial paralysis recovered gradually from 6/40 up to 30/40 points of Japanese grading system. Tumor removal together with facial nerve decompression were thought to be effective for facial nerve regeneration.
