HIV感染血友病患者に発生した顔面神経麻痺

A Case of Facial Paralysis Concomitant with HIV Infection

　はじめに

　近年，わが国においてもヒト免疫不全ウイルス（HIV）感染者が急速に増加する傾向にある。HIV感染者，あるいはAIDS患者の耳鼻咽喉科領域の併発症状としては，口腔カンジダ，口腔粘膜・頭頸部皮膚のカポジ肉腫，咳，などが一般的に知られているが末梢性顔面神経麻痺も頻度は少ないものの注意すべき症状の1つである^1〜7）。最近，血友病患者で血液製剤によりHIV感染をきたし，経過中に一側末梢性の顔面神経麻痺を発症した症例を経験した。われわれの渉猟した限りでは，本邦においてHIV感染者に顔面神経麻痺が発症したとの報告はない。HIV感染者に発症する顔面神経麻痺について文献的考察を加え報告するとともに，MRIおよびウイルス血清学的に顔面神経麻痺の病因を考察した。

A 23- year old female with hemophilia B became positive HIV by repeated administration of platelet factor 9. Left facial palsy developed suddenly with-out any other neurological signs and was incomplete graded as 24/40 point. The facial palsy gradually improved and completely healed within eight weeks. Gadlinium enhancement was manifested in the left geniculate ganglion by MRI. Additionally, herpes simplex and cytomegalo virus antibody titers chan-ged between acute and convalescent phase. A geniculate ganglionitis induced by this virus was speculated as a cause of facial palsy, complicating with immunodeficiency, even though a possibility of direct infection of HIV itself could not be denied.
