A 61year-old-female presented with a history of sudden hearing loss of one year's duration. A tumor mainly located within the internal auditory meatus was found by MRI. Her preoperative pure tone audiogram and speech discrimination score were 74 dB and 5%, respectively. Post-operatively these were improved to 43 dB and 55% respectively. This case raises a question against the so-called 50-50 rule as the preoperative criteria for hearing preser-vation of acoustic neuroma surgery.
