I．はじめに

　microsurgeryの導入，手術テクニックの進歩により聴神経腫瘍の手術は極めて安全に行われるようになり，腫瘍全摘出および顔面神経温存に関してすぐれた成績が報告されてきた^{14,17,18,26,28,30,33）}．しかるに，最近では欧米を中心に直径1-2cmの小さな聴神経腫瘍の手術成績・聴力温存の報告がなされつつある^{1-3,9,12,13,17,23,28,33,32）}．本邦においては，現在でも聴神経腫瘍を論じる際には径2-3cm以上の大きな腫瘍の議論が中心であり，聴力温存の報告は少ない^30）．われわれは過去5年間に43例の聴神経腫瘍に対し手術を行ったが，術前聴力が保存されている症例を10例経験した．そのうち腫瘍の大きさが4cm未満であった6例について聴力を温存するよう試みた．今回はこの結果を報告し，術前にはどのような症例で聴力が保存されており，手術に際していかなる点に注意すれば聴力を保存しうるかについて考察したい．

Six cases of acoustic neurinoma (AT) with preoper-atively preserved hearing are presented. Their clinical features and surgical management for hearing preserva-tion are discussed.

Presenting symptoms were hearing decrease in 3 cases ( # 1, # 2, # 6), trigeminal neuralgia in 3 cases ( # 4, # 5, # 6), and hemifacial spasm in case # 2. Case # 3 was an incidentally diagnosed case by CT scan. Preoperative decrease of hearing in the affected ears ranged from 10 dB to 60 dB. In all cases except for case # 1, CT scans revealed CP angle tumors sized 1 - 4 cm in diameter.
