眼窩内偽嚢胞を伴った篩骨ならびに上顎嚢胞の1症例

Intraorbital Pseudocyst Combined with Independent Ethmoidal and Maxillary Mucoceles

　はじめに

　副鼻腔炎または副鼻腔膿嚢胞に伴う鼻性眼窩内合併症は，一般にその経過が急性であり，その眼症状は重篤となる。今回の報告例は症状が軽微でありながら，多発性副鼻腔嚢胞に眼窩内偽嚢胞を併発したものである。各々の嚢胞内容液の生化学分析およびMRIにおける緩和時間の測定を併せて行った。症例の報告とともに，本症例における各嚢胞の成因に関する考察を試みた。

A 61-year-old female presented with an intra-orbital pseudocyst and primary mucoceles in the ethmoid and the maxilla. The orbital cyst was located at the intraorbital subperiosteal layer and revealed no epithelial lining, and showed a high signal intensity on T1-weighted MR images. The content turned out to have high protein, iron and cholesterol concentration by biochemical assays. The result was different from that of primary paranasal mucoceles.

The intraorbital pseudocyst was possibly orig-inated through an intraorbital subperiosteal abscess caused by an acute inflammatory reaction of the ethmoidal mucocele and the whole process did not show any acute distress.
