副鼻腔嚢胞重複症例の検討—とくに術後性篩骨嚢胞について

Combination of Postopertive Ethmoidal Mucoceles with Other Sinal Mucoceles:A Report of 5 Cases

　はじめに

　日常診療において，同一症例の異なる副鼻腔に嚢胞が重複して存在する症例を経験することが少なくないが，その検討を行った報告は少ない。我我は昭和60年から平成元年までに術後性上顎嚢胞に篩骨嚢胞を合併した5症例を経験したので報告し，ついで篩骨嚢胞を中心に副鼻腔嚢胞重複症例について検討する。

Five cases of postopertive ethmoidal and maxil-lary mucoceles were reported. A summary of sinal mucoceles experienced at Tokyo University Branch Hospital from 1985 to 1989 was done, and 14 ethmoidal mucoceles (13 postoperative, one pri-mary) were studied. All postoperative ethmoidal mucoceles were combined with other sinal muco-celes. The combination with postopertive maxillary mucocele was high in incidence, about 40% of our ethmoidal mucoceles. When a mucocele of a certain paranasal sinus is diagnosed both by clinical and imaging modalities, other pranasal sinuses should be included in exploration by CT and/or MRI, to rule out concomitant mucoceles which may lead to either intracranial or orbital complica-tions.
