ハイドロキシアパタイト耳小骨による連鎖再建術の検討

Reconstruction of Ossicular Chain with Hydroxyapatite Prosthesis

　はじめに

　鼓室形成術における耳小骨連鎖再建に人工耳小骨を使用する場合，組織親和性，生体安定性に優れることが要求される。最近，登場した無機材料のセラミック人工耳小骨は組織親和性に優れ，これまでの報告でも有用^1〜7）とするものが多い。当教室でも1987年12月より一部の症例の耳小骨連鎖再建にハイドロキシアパタイト耳小骨を使用している。今回は術後聴力と素材としての安定性を中心にその有用性を検討したので報告する。

From December 1987 through December 1991, tympanoplasty using hydroxyapatite ossicular prosthesis was performed and postoperative follow-up for more than six months was con-ducted in 29 ears ; 15 ears with conductive hear-ing loss with intact tympanic membrane, 10 cholesteatoma and 4 chronic otitis media.

The analysis on these cases revealed that 89.7 % of the ears satisfied the criteria of success according to classification recommended by Otological Society of Japan.

All 15 ears with conductive hearing loss with intact tympanic membrane were judged as sucess according to this criteria, while in ears with cholesteatoma or chronic otitis media success rates were lower.
