外来で行う保存的鼓膜形成術—術式の改良と聴力改善について

An Office Technique of Conservative Myringoplasty Using Lyophylized Dura and Fibrin Glue

　はじめに

　通常の鼓膜形成術（Myringoplasty）のかわりに，シガレットペーパーやステリストリップなどを用いる保存的鼓膜閉鎖法（office closure method）に準じた手術手技で，グラフトを生着させる術式であるならば，手術侵襲も少なく外来での手術が可能である。われわれは従来より組織接着剤と同種乾燥硬膜（lyophylized homograft dura，以下LHD）を用いて，外来にて鼓膜を閉鎖形成すること（保存的鼓膜形成術，conservative myringo-plasty）を試みている^1,2）。今回は過去3年間の手術症例数が243例に達し，聴力を中心とした術後成績を検討したので，その成績に検討を加え，特に術前術後の聴力改善の効果を検討したので報告する。

A new office technique of conservative myringoplasty using lyophylized homograft dura and fibrin glue was carried out on 243 ears to close central perforations of the tympanic mem-brane. These cases who underwent this tech-nique revealed 69.9% closure rate intially and 84.9% success rate finally after repeated pro-cedures. This conservative myringoplasty is re-commended for patients, whose middle ear has no or minimal pathology, because it is simple and times-and money-saving.
