中耳腔内を異常走行する内頸動脈の1例

Aberrant Intratympanic Internal Carotid Artery

　はじめに

　側頭骨内の血管奇形の中で動脈奇形は極めて稀な疾患である。これらの疾患の主訴は難聴，耳鳴，耳閉感などであり，日常の耳鼻咽喉科診療においてありふれた症状といえる。しかし，本疾患は誤診や不用意な外科的処置により大出血や神経学的合併症を認めた症例も数多く報告されており，臨床的に非常に重要な疾患である。

　今回，われわれは拍動性耳鳴を主訴に来院した24歳の女性で初診時にグロームス腫瘍を疑い，術前検査で内頸動脈の走行異常と診断し，シリコンシートを用いることによりその拍動性耳鳴を軽減できた症例を経験した。われわれの経験した1症例とともに，現在までに国内外で報告されている内頸動脈異常走行例36症例^1〜16）と併せて37症例について検討したので報告する。

A 24-year-old woman presented with pulsatile tinnitus of her left ear. The diagnosis was glomus jugulare tumor. However high resolusion CT showed a soft mass density of 5 mm in diameter in the bottom of the left tympanic cavity. Angiography of The external carotid artery showed the aberrant internal carotid artery in the tympanic cavity. Under an exploratory tympa-notomy, the artery was covered with a silicon sheet, and the patient has partiially been relieved from the pulsatile tinnitus.
