A right tortuous spiral-appearing extracranial in-teral carotid artery was found in a 42-year-old woman during examination for neurological symp-toms following a car accident. Arteriographic study and the operative findings revealed that the lesion began 8 mm distal to the right carotid bifurcation for a length of 3 cm, showing 6 clockwise spiral twist. The blood flow in the artery seemed to be impeded, but not completely obstructed. There were no any arteriosclerotic or inflammatory changes of the blood vessel. No pathology suggesting relation-ship with the accident was found. The underlying nature of the blood vessel seemed to be congenital.

Perivascular sympathectomy of the right carotid and vertebral arteries improved neurological symp-toms, dizziness, intentional tremor of left upper extremity.
