頸部内頸動脈屈曲異常例

Abnormally Twisted Cervical Internal Carotid Artery

I．はじめに

　頭蓋外血管の異常により惹起される脳血流障害は，欧米ではほとんど頸部内頸動脈および椎骨動脈の動脈硬化によるものが多く，またしばしば，内頸動脈の屈曲，蛇行などの変形により内腔の閉塞および狭窄などがみられる^1）2）4）。これら疾患に対し，血管再建術などの方法が試みられ，よい成績が報告されている。しかしながらわが国にはかかる症例は少ない。

　最近われわれは交通事故患者の検査中，偶然頸部内頸動脈がコイルスプリング状に屈曲している症例をみつけかかる異常が脳血流障害に関与すると考えられた症例を経験したので報告する。

A right tortuous spiral-appearing extracranial in-teral carotid artery was found in a 42-year-old woman during examination for neurological symp-toms following a car accident. Arteriographic study and the operative findings revealed that the lesion began 8 mm distal to the right carotid bifurcation for a length of 3 cm, showing 6 clockwise spiral twist. The blood flow in the artery seemed to be impeded, but not completely obstructed. There were no any arteriosclerotic or inflammatory changes of the blood vessel. No pathology suggesting relation-ship with the accident was found. The underlying nature of the blood vessel seemed to be congenital.

Perivascular sympathectomy of the right carotid and vertebral arteries improved neurological symp-toms, dizziness, intentional tremor of left upper extremity.
