 An aberrant internal carotid artery is a rare vascular malformation. Patients with this anomaly may be asymptomatic or may complain of hearing loss or pulsatile tinnitus.

 We present the case of a 67-year-old woman who complained of pulsatile tinnitus caused by an aberrant internal carotid artery. If a myringotomy is performed without an accurate diagnosis of this malformation, it may lead to life-threatening complications. Otolaryngologists should keep in mind the possibility of a vascular malformation when they contemplate middle ear surgery.
