鼓膜穿孔を伴わない巨大真珠腫の1例

Cholesteatoma without Perforation of the Tympanic Membrane:Report of a Case

はじめに

　難聴や耳鳴などの蝸牛症状を伴う，めまい・平衡障害で，さらに，顔面神経麻痺や小脳症状などを伴う症例に遭遇すると，私達はまず，小脳橋角部腫瘍，特に，聴神経腫瘍を疑う。可能性は少ないが，真珠腫の巨大なものの存在も思い浮かべるものの，中耳炎の既往もなく，鼓膜にも穿孔も癒着も存在しない場合は，これも否定しがちである。

　今回，私達は上記のような症例を経験した。本症例では，シューラー法やCT scanで，大きな側頭骨部腫瘤の存在を疑わせる陰影を認めた。しかし，これだけでは，腫瘤と，後頭蓋窩，中頭蓋窩の脳組織との連続性を否定しきれなかったので，Magnetic Resonance Imaging （以下，MRI），Digital Subtraction Angiography （以下，DSA）を行い，脳組織との連続性は否定的との画像診断を得た。この診断を根拠に，経乳突部手術で摘出された腫瘤は，真珠腫であった。

A 72-year-old woman was suffering from diz-ziness, tinnitus, hearing loss and facial nerve palsy. Her tympanic membrane had no perforation. She had never been suffering from otitis media before. X-ray examinations, CT scan and magnetic reso-nance imaging of the brain and the temporal bone indicated that there was a large mass in her left temporal bone, which pressed the middle cranial fossa and posterior cranial fossa, and the mass seemed to exist between them and the dura mater. This mass was removed by transmastoid approach. The histological examination revealed a cholesteatoma behind the intact tympanic membrane.
