A 72-year-old woman was suffering from diz-ziness, tinnitus, hearing loss and facial nerve palsy. Her tympanic membrane had no perforation. She had never been suffering from otitis media before. X-ray examinations, CT scan and magnetic reso-nance imaging of the brain and the temporal bone indicated that there was a large mass in her left temporal bone, which pressed the middle cranial fossa and posterior cranial fossa, and the mass seemed to exist between them and the dura mater. This mass was removed by transmastoid approach. The histological examination revealed a cholesteatoma behind the intact tympanic membrane.
