Crocodile Tears症候群11例の臨床的考察

A Clinical Analysis of 11 Patients with Crocodile Tears Syndrome

はじめに

　Crocodile Tears症候群は，食事の時などに一側性に涙が異常に出る現象であり，まれなものとされている。これは，1400年頃の小説「The voiage and travaile of sir John Maundeville，kt．」の中のワニが獲物を食べる時に偽善的に涙を流すという話より名付けられたものといわれており，1913年Oppenheimにより報告され，1928年Bogoradによりsyndrome of Crocodile Tears （Krokodil—stranen Syndrome）と命名された^1）。

　この症状は末梢性顔面神経麻痺後にみられることが多く，いわゆるBell麻痺，Ramsay Hunt症候群，外傷性顔面神経麻痺などの後遺症として発症することが報告されている^2）。今回，上記疾患の他に聴神経腫瘍摘出術後の症例を加えて計11例のCrocodile Tears症候群について若干の文献的考察を加えて報告する。

Crocodile Tears syndrome frequently occurs after peripheral facial nerve palsy. Eleven cases of Crocodile Tears including 5 cases occured following peripheral facial nerve palsy and 6 cases following surgery for acoustic tumor by the translabyrinthine approach were investigated.

All symptoms appeared after facial nerve palsy caused by the lesions in the internal auditory meatus. This suggests that misdirection occurs more easily between the intermediate nerve and the greater petrosal nerve which run parallel in the internal auditory meatus.

The onset of Crocodile Tears syndrome was after 3 to 12 months in cases of peripheral facial nerve palsy and within 5 months follwoing operation for acoustic tumor.
