めまい・平衡障害を伴う突発性難聴の1症例—特に眼振所見を中心とする観察と文献的考按

Sudden Deafness Accompanying Vertigo : Report of a case

はじめに

　めまい・平衡障害を伴う突発性難聴は，厚生省研究班によって報告された，突発性難聴診断の手引き（診断基準）^1）によって定義される突発性難聴に，原則として1回のみのめまい発作を伴う内耳障害疾患であるとされている。厚生省前庭機能異常調査研究班によると，メニエール病，良性発作性頭位めまい症に次いで多いとされている前庭機能異常疾患でもある^2）。

　しかし，本疾患の難聴・耳鳴などの蝸牛症状が難治性であるのに対して，大きなめまい発作は通常1回であり，平衡障害も，中枢神経系の代償作用により多くの症例では1ヵ月以内に自覚症状が軽快するので，本疾患の前庭症状については蝸牛症状ほど詳細に研究されていない。

A 23-year-old female suffered from suddendeafness of the right ear accomanying vertigo. From the beginning of the attack of vertigo, we recorded her nystagmus on ENG. As a result, at the beginning the nystagmus beated to the rightside, then to the left side. Therefore, we specula-ted that the etiology of this case might be a kind of inflammation.
