メトトレキサート関連リンパ増殖性疾患におけるFDG-PET/CTの有用性

The usefulness of FDG-PET/CT to diagnose patients with MTX-LPD

関節リウマチ治療薬のメトトレキサート（methotrexate：MTX）服用中に発症するリンパ腫を含めたリンパ増殖性疾患［MTX関連リンパ増殖性疾患（MTX-associated lymphoproliferative disorders：MTX-LPD）］は医原性免疫不全関連のリンパ増殖性疾患の一つで，低用量MTX治療中に発症する^1-3）。我が国では近年増加傾向であり，一部は死亡に至る重篤な合併症として認識されている。MTX-LPDはMTX休止のみにより20～70％の症例で消退が認められるとされる^4-6）。

We reviewed the usefulness of FDG-PET/CT to diagnose 8 patients with MTX-LPD. Five of 8 patients（62.5％）had extranodal focis. CMR by discontinuation of MTX obtained in 3 of 5 patients（60.0％）with first line treatment and 3 of 4 patients（75.0％）at final observation. With all treatments 6 out of 8 patients（75.0％）had CMR and 2 of 8 patients（25.0％）had PMR at final observation. Recurrence was observed 4 of 8 patients（50.0％）. We considered FDG-PET/CT to be useful in the diagnosis of patients with MTX-LPD.
