Actinomyces odontolyticusによる急性型頸部放線菌症の1例

Acute cervical actinomycosis due to Actinomyces odontolyticus

はじめに

　頸部放線菌症は口腔内に常在する嫌気性グラム陽性桿菌のActinomyces属による感染症である。主な原因菌はActinomyces israeliiであり，慢性型の経過を示す例が多いが，稀にほかの菌種による急性型の頸部放線菌症がみられる^1〜3）。今回われわれは，Actinomyces odontolyticusが原因菌となった急性型頸部放線菌症の1例を経験したので報告する。

　We reported a rare case of acute actinomycosis of the left neck in a 72-year-old male visited our hospital with a swelling and pain of the left neck after dental work. CT and MRI indicated an abscess with gas-formation in that area. Surgical drainage was performed under general anesthesia and a large amount of sulfur granules were found in the abscess. Culture of the abscess grew out Actinomyces odontolyticus. Histopathological examination revealed actinomycosis. After operation, penicillin was administrated and local irrigation was done. Clinical course was satisfactory until now.
