当科における喉頭癌治療例の臨床的検討

Clinical study of 76 cases of laryngeal cancer

Ⅰ．はじめに

　当院は，地方の一地域における基幹病院として地域医療を担ってきた。地域癌診療連携拠点病院や頭頸部癌専門医制度認定研修施設には認定されていないが，放射線治療装置を有しており，頭頸部癌に対しても積極的に治療を行ってきた。

　今回われわれは，過去10年間に当科で経験した喉頭癌新鮮例に対してその実状を分析し，今後の喉頭癌診療の一助とするため臨床的検討を行ったので報告する。

　We evaluated the outcome of 76 cases of laryngeal cancer primarily treated in our hospital from January 2001 to December 2010. Supraglottic cancer was found in 21 cases（27.6％）,glottic cancer in 54 cases（71.1％）,and subglottic cancer in 1 case（1.3％）. The five-year survival rates were as a whole 82.9％,supuraglottic type 57.2％,glottic type 92.9％,and stage Ⅰ 100％,stage Ⅱ 83.6％,stage Ⅲ 57.1％,stage Ⅳ 35.7％,respectively. The five-year organ preservation rates were 82.4％ in T1,61.4％ in T2,respectively. Concurrent chemoradiotherapy is important for improvement of survival and organ preservation of laryngeal cancer.
