Nerve integrity monitor（NIM）による顔面神経モニタリングで反応が得られなかった耳下腺手術症例

A case of parotid gland surgery in which facial nerve monitoring by nerve integrity monitor（NIM）was unuseful

はじめに

　Nerve integrity monitor（NIM）は，電気生理学的メカニズムによる筋収縮を指標に運動神経の同定を行うための機器で，昨今頭頸部領域での手術に頻用されている。筋強直性ジストロフィー症例は多系統疾患であり，全身麻酔の術中および周術期に循環器系，呼吸器系などにおいて多くの注意すべき点が指摘されている^1）。そのため，筋強直性ジストロフィー患者の手術報告自体は散見される。今回われわれは，術前に筋強直性ジストロフィーが疑われた症例で，NIMによる術中顔面神経モニタリングで反応が得られなかった1例について若干の文献的考察を加えて報告する。

　We presented a parotid gland surgery case in a patient with myotonic dystrophy that a nerve integrity monitor（NIM）was not useful. In the patient with myotonic dystrophy, muscle relaxant action of rocuronium has been known. We thought why NIM didn't work in this case, because the patient have no peripheral nerve palsy and no instrumental malfunction. We should note that in a case with myotonic dystrophy NIM may not be useful.
