下鼻甲介に発生した骨血管腫の1例

A case of intraosseous hemangioma arising from the inferior turbinate

はじめに

　骨に発生する血管腫である骨血管腫は骨腫瘍の0.7％を占め，その約3分の2は脊椎骨や頭蓋骨から発生する^1,2）。頭蓋骨のなかでは下顎骨が最も多く，鼻骨，頰骨，上顎骨などの報告^3～5）も散見されるが，鼻甲介骨から発生することは稀である^6）。今回，われわれは下鼻甲介に発生し，鼻涙管狭窄をきたした骨血管腫の1例を経験したので若干の文献的考察を加えて報告する。

　We reported a rare case of intraosseous hemangioma arising from the right inferior turbinate. A 53-year-old weman visited our hospital with the chief complaint of right hyperlacrimation and nasal obstruction. Anterior rhinoscopic examination revealed a smooth surfaced, light-red colored and stony hard mass obstructing the right nasal cavity. In CT scan and MRI, an expansive mass was located in the right inferior turbinate and the content of the mass looked like honeycomb appearance. Under general anesthesia, the patient underwent the surgical excision, and the mass was removed by piecemeal with the endoscopic endonasal approach. Diagnosis of the intraosseous cavernous hemangioma was obtained by the histopathological examination. No reccurences was observed after the surgery until now.
