小児良性発作性めまいの2症例

Two cases of benign paroxysmal vertigo of childhood

はじめに

　めまい患者は一般に高齢者に多く，小児では少ない。小児のめまいでは患児がめまいの性状を正確に表現することができないため，また平衡検査などの検査も十分に協力が得られないため，診断に苦慮することが多い。これまで小児めまいの原因は，教科書的に起立性調節障害，頭痛に関連しためまい，心因性めまいなどが大部分であるとされてきた^1）。しかしなかには脳腫瘍などの重大な疾患が潜んでいる可能性もあり，診断には注意を要する。

　近年，小児良性発作性めまい（benign paroxysmal vertigo of childhood：BPV）が小児めまいの原因として注目され，報告されるようになってきた。われわれは最近，小児良性発作性めまいの2例を経験したので，文献的考察を加えて報告する。

　We reported two cases of benign paroxysmal vertigo of childhood（BPV）. The first was a 4-year-old boy, and the second was a 10-year-old girl. We analysed the concept of BPV by reviewing 33 cases of BPV in Japanese literature, and compared our cases with the analysed results. As recognition of BPV is lacking in otolaryngologists as well as pediatricians, it is necessary to enlighten them regarding this disease.
