上咽頭癌治療における手術療法の役割

Treatment of Nasopharyngeal Carcinoma: Surgery combined with Radiation Therapy

Ｉ．はじめに

　上咽頭癌は，1）低分化型の扁平上皮癌やリンパ上皮腫といった放射線感受性の高い腫瘍が多い，2）発生部位が深部にあり解剖学的に複雑なために外科的治療の適応となりにくい，3）難聴，鼻出血など隣接臓器の症状を呈するまで無症状に経過することが多い，4）原疾患の早期から頸部リンパ節転移，遠隔転移がしばしばみられる，5）細胞性免疫能の低下が著しい，などの特徴がある。したがって現在では上咽頭癌の治療は放射線療法が主体となっているが，その5年生存率は30〜40％以下であり他の頭頸部癌と比較して極端に低い^1〜4）。本疾患の治療成績の向上のために遠隔転移防止の観点から化学療法，免疫療法の重要性が強調されている。一方原発巣が深部にあるために従来の理学的診察では病巣の広がりを過小に評価しがちである^5〜7）。また根治照射終了時の腫瘍の残存を診断しえない例も少なくない。しかし最近のCT検査の発達によって原発巣の診断および経過の追跡が正確に行えるようになった^8,9）。その結果根治照射終了後上咽頭ファイバースコピーでは残存腫瘍を認めず従来なら治癒と判断された例でも，CT検査によって腫瘍の残存を見出しうる症例を少なからず経験した。このような症例に対し著者らは積極的に外科的治療を追加し治療成績の向上に努力してきた。本稿では過去4年間に当科で経験した9例の上咽頭癌の治療について報告し，併せて上咽頭癌治療における手術治療の役割について考察した。

CT scans arc able to reveal the extent of car-cinoma of the nasopharynx (NPC) clearly, andthe correct area to be treated is easily deter-mined. Radiation therapy has been and still is the primary treatment for NPC at the authors' institution and most of others. However, CT scans showed that NPC did not always disap-pear after 60 Gy irradiation. We performed sur-gical treatment to eradicate such residual tumor through a transpterygoid approach. Four patients were operated on by this method. One of them died of multiple metastases while others are alive with no evidence of disease. Surgical technique and complications of this operation were presented.
