頸長筋に達した上咽頭腔外異物の1症例

A one-year-old case of a foreign body outside the pharynx, undetected just after the injury

はじめに

　咽頭異物は日常診療でよく接する疾患である。異物の種類としては，わが国では魚骨が大半を占め，刺入部位としては口蓋扁桃が多い。また，高齢者においては義歯やPTP包装シートの誤飲が問題となり，時に全身麻酔下での摘出を要することがある。

　小児や高齢者の場合，症状や訴えがわかりにくいことがあるうえに，いったん粘膜下に刺入埋没すると確認が困難となり画像診断が必要となる^1）。

　今回，われわれは受傷直後の画像診断において，異物同定が困難であったが経時的なCT施行により上咽頭腔外の異物の診断に至り摘出した小児症例を経験した。文献的考察も併せて報告する。

　In general the diagnosis of foreign bodies outside the pharynx is difficult. We report a one-year-old case of a foreign body outside the pharynx, which couldn't be detected just after the injury. Repeated CT scan could lead us to detect the foreign body, and we removed it safely in trans-oral surgery with CT Navigation system, instead of an external skin incision. CT Navigation system is useful for removal of the foreign body outside the pharynx.
