鼻口蓋管囊胞の1例

Nasopalatine duct cyst：A case report

はじめに

　鼻口蓋管囊胞は1914年にMayer^1）により初めて報告された，比較的稀な発育性の非歯原性囊胞である。

　本囊胞は多くの症例が上顎正中部に発生し，胎生期に存在し生後1年以内に消退する鼻口蓋管の遺残上皮に由来する^1〜3）。発生原因については遺残上皮への口腔内からの感染の波及，中切歯根の根端部に生じた炎症の波及，不適合義歯，咀嚼による刺激などに起因する外傷などが誘引となり症状を呈する^4,5）。

　今回われわれは上顎正中部から上顎洞や鼻腔，口蓋を圧排するように増大・発育している巨大な鼻口蓋管囊胞を経験したので，若干の文献的考察を加えて報告する。

　Nasopalatine duct cysts are relatively rare non-odontogenic developmental cysts.

　In the current case, we found an exceptionally large nasopalatine duct cyst, which had grown and developed as if to displace the periphery from the maxillary mid-portion.

　The case was a 38-year-old male with chief complaints of a swelling and pain of the left cheek. Based on the overall findings, including clinical presentations and image findings, we diagnosed the case as a nasopalatine duct cyst.

　As treatment, we chose total removal. Postoperatively, the patient showed no sign of relapse and his QOL has been maintained.
