巨大耳下腺多形腺腫の1例

A case of gigantic pleomorphic adenoma of the parotid gland

Ⅰ．はじめに

　耳下腺腫瘍の中で多形腺腫は最も頻度の高い腫瘍である。病理学的には良性腫瘍であり，発育も緩徐であるが，長期経過中にて悪性化する場合があることも知られている。また，近年では比較的早期に手術を施行される傾向にあると思われる^1,2,23）。一方，医療技術の進歩により高齢者に対しても手術を行うようになってきており，手術適応に対する考え方も変わりつつある。今回，われわれは発症より約30年放置されていた高齢者の巨大耳下腺多形腺腫の手術例を経験したので，文献的考察を加えて報告する。

　An 89-year-old woman who had a large mass in the left parotid region since about 30 years visited a community hospital because the mass had gradually enlarged and disintegrated. Computed tomography（CT）revealed a gigantic parotid tumor,and she was referred to our hospital for its surgical removal.

　Histopathologically,the tumor was diagnosed as pleomorphic adenoma without evidence of malignancy. The extirpated tumor was 13 cm in diameter and weighed 630 g.
