肺転移をきたした耳下腺多形腺腫例

A Case of Parotid Pleomorphic Adenoma with Lung Metastasis

I．はじめに

　多形腺腫は，唾液腺腫瘍の中で発生頻度の最も高い良性腫瘍である。しかし，被膜形成の不十分な腫瘍では再発をきたしやすく，長期間放置された症例では悪性化をきたすことも知られている^1）。さらに，極めて稀ではあるが組織学的には良性を示すにもかかわらず，肺や骨に遠隔転移をきたす症例の報告^1）もあり，臨床的には低悪性度腫瘍に準じた対応が必要である。

　今回，われわれは耳下腺多形腺腫の治療後に肺転移をきたした症例を経験したので，若干の文献的考察を加えて報告する。

　A 71-year-old woman was operated on for a lung tumor 4 years after parotid gland surgery. The histological examinations of the lung tumor showed benign pleomorphic adenoma and the findings were as similar as that of a primarily operated parotid tumor.

　The literature about metastasizing pleomorphic adenoma was reviewed and the mechanism of metastasis was discussed.
