広範な自然壊死を伴った耳下腺多形腺腫例

A case of pleomorphic adenoma with extensive spontaneous necrosis

はじめに

　疼痛，急激な腫脹，顔面神経麻痺は，耳下腺悪性腫瘍を疑う3大徴候である。ワルチン腫瘍では炎症により急激な増大を示す場合があるが，多形腺腫では急激な腫脹や痛みを伴うことは珍しい^1）。一方，耳下腺良性腫瘍に穿刺吸引細胞診を繰り返すことで内部壊死を引き起こし，続発的に疼痛や腫脹を呈することが報告されているが，自然壊死を生じることはきわめて稀である^2）。今回，右耳下部の急激な腫脹と疼痛を主訴に受診し，腫瘍内部に広範な自然壊死を呈した耳下腺多形腺腫例を経験したので報告する。

　Pleomorphic adenomas of the parotid gland rarely cause pain or swelling. We report the case of a 55-year-old man with a parotid gland pleomorphic adenoma and acute and painful swelling. The MRI revealed a parotid gland tumor with extensive internal necrosis. The tumor was completely removed without any side effects. The histopathological findings revealed broadly pleomorphic adenomas with extensive central necrosis. Necrosis of the parotid gland following fine-needle aspiration biopsy has been reported in benign and malignant tumors. However, pleomorphic adenomas with spontaneous necrosis, as observed in the present case, are extremely rare.
