経鼻内視鏡下切除術を施行した髄膜瘤の幼児症例

Endoscopic endonasal approach for a 2-year-old infant with a transsphenoidal meningocele

Ⅰ．はじめに

　脳瘤は先天的な頭蓋骨の欠損孔から頭蓋内容が頭蓋外に脱出して囊瘤を形成した疾患で，頭瘤とも呼ばれる。脳瘤の内容が髄膜と髄液だけであれば髄膜瘤（meningocele），その中に頭蓋腔から逸脱した脳組織が含まれる場合を髄膜脳瘤（meningo-encephalocele）と呼ぶ^1）。発生頻度は10,000出生に対して1～2で，発生部位は頭蓋冠，特に後頭部に多いが，頭蓋底から鼻腔に脱出するものも5～20％の発生率でみられる。

　鼻腔に脱出した脳瘤に対する手術のアプローチとしては従来，経頭蓋法，経口・経口蓋法が多く報告されていたが^2～7），近年では内視鏡を用いた経鼻法により良好な結果を得たとする報告もみられる^8～12）。反復する髄膜炎を契機に発見され，頭蓋咽頭管遺残から鼻腔へと脱出したと考えられる髄膜瘤に対して経鼻内視鏡下切除術を施行した幼児の1例を経験したので報告する。

　Use of the transcranial and transoral approaches has mainly been reported to treat symptomatic transsphenoidal meningoceles,but some recent studies stated that the endoscopic transnasal approach is superior to these approaches because of its high success rate,minimal invasion,and excellent visualization.

　In a 2-year-old girl who had a transsphenoidal meningocele and the chief complaint of recurrent meningitis,the endoscopic transnasal approach was performed. The patient has no complications 6 months after the surgery. Although the patient was an infant,the endoscopic transnasal approach not only provided clear visualization but also decreased complications.
