頸部軟部組織内を移動し摘出に難渋した魚骨異物の症例

A migrated fish bone through soft tissue in the neck after penetration of middle pharyngeal mucosa

Ⅰ．はじめに

　咽喉頭異物は耳鼻咽喉科外来診療にてしばしば経験する疾患である。特に魚骨異物は，骨のついた状態で魚料理を供する習慣のあるアジア圏での報告がほかの地域と比べて多い^1）。異物は扁桃や舌根に存在することが多いため，大部分は直視下または咽喉頭内視鏡下に摘出が可能である^2）が，稀に軟部組織内に侵入してしまい摘出に苦慮することがある。今回われわれは，咽頭粘膜に刺入し頸部軟部組織内を移動し摘出に苦慮した魚骨異物の症例を経験したので報告する。

　Ingested foreign bodies are common otorhinolaryngological emergencies,most notably fish bones. These foreign bodies usually are easily removed via a direct endoscope. But when it migrates into soft tissue,detection and its removal become much difficult. We reported a rare case of 2-cm fish bone which was so thin and sharp like needle that it migrated through soft tissue in the neck after penetration of middle pharyngeal mucosa. The bone was detected only by high resolution computed tomography（CT）. We removed it surgically with lateral neck incision immediately after it was located by CT.
