呼吸困難にて治療を要した乳児舌根部囊胞の1例

Ⅰ　はじめに

　舌根部囊胞は舌根部にできる囊胞の総称であり，舌根部甲状舌管囊胞，舌根部貯留囊胞などが含まれる。小児舌根部囊胞は，成人と比較し頻度が低いものの，時に咽頭部の狭窄による呼吸障害，突然死の原因となる可能性があり，適切な対応が必要である。舌根部囊胞の治療法については，早期手術か，経過観察かといった治療のタイミングや手術法の選択，術後気管切開が必要か早期抜管可能かなど，特に小児においてはさまざまな見解がある。

　今回われわれは呼吸困難で発症した乳児舌根部囊胞を経験したのでその診断，治療法の選択について文献的考察を加え報告する。

　Lingual thyroglossal duct cyst is a rare disease in infants,but it is important for differential diagnosis of upper airway obstruction. A one-month-old male infant was referred to our hospital because of inspiratory stridors and sleep apnea. Endoscopic and radiographic examinations revealed a cystic mass in the base of tongue. Partial resection of the cyst wall was performed under general anesthesia. Postoperative course was uneventful and the respiratory distress was dramatically improved. No recurrence has been observed during the one-year follow-up period.
