We reported the first case of metastatic renal cell carcinoma to the temporal fossa 14 years after tumor nephrectomy. A 59-year-old male had been aware of a swelling mass at the right temporal region for 3 years. Under a diagnosis of metastatic renal cell carcinoma, surgery was performed with facial dismasking approach. There was no apparent cosmetic complications including facial palsy. The patient was currently free from disease one year after surgery. The facial dismasking approach leads to excellent access for resection of a tumor involving the temporal fossa with minimum damage to facial skin and well-preserved facial movement.
