眼窩転移で発見された腎細胞癌

A Case of Renal Cell Carcinoma Found by Intraorbital Metastasis

　73歳，男性。複視，眼球突出を主訴に近医を受診した。左眼窩内に腫瘤を認めたため吸引細胞診を施行したところ，class．IVとの結果であった。また，全身の精査にて左腎細胞癌，左眼窩転移，多発性肺転移，右鎖骨窩リンパ節転移の診断で根治的腎摘除術を施行した。術後インターフェロンαの投与，眼窩転移に対して塞栓術を施行したが，腫瘍は口腔内にまで増大し，肺炎の合併，全身状態の悪化を認めて死亡した。

A 73-year-old man presented with a 18-month history of diplopia and exophthalmos. Enlarged lymph nodes were palpable in the left supraclavicular region. CT and MRI showed 2.5 × 3 cm intraorbital mass, left renal mass 5 cm in diameter and multiple pulmonary metastasis. Biopsy of a supraclavicular node and aspiration cytology of a intra-orbital mass showed clear adenocarcinoma cells compatible with a metastasis from renal cell carcinoma. Left radical nephrectomy was performed.
