胸腔鏡下に摘出し得た結腸癌胸壁転移の1例

Thoracoscopic resection for thoracic metastasis of colonic carcinoma:A case report

　患者は71歳，男性，横行結腸癌根治術後16か月目に，右胸壁腫瘍と転移性肝腫瘍に対して胸腔鏡下胸壁腫瘍摘出術および開腹下肝部分切除術を施行した．径3cm大の胸壁腫瘍は壁側胸膜に覆われており，肋骨や肋間筋への浸潤は認めず完全摘出できた．病理組織学的には胸壁腫瘍，肝腫瘍ともに横行結腸癌からの転移と診断された．左胸郭形成術の既往のため高度の換気障害を有していたが，経過良好で術後第11病日に退院した．転移性胸壁腫瘍に対する胸腔鏡下手術のわが国における報告例は比較的稀であるが，低肺機能症例に対しても安全に施行可能であり，胸腔内病変に対する審査胸腔鏡という面のみならず低侵襲性という面できわめて有用であると考えられた．

The patient was a 71-year-old man. Two tumors in the right chest wall and liver were found sixteen months after a radical operation for transverse colon cancer. Thoracoscopic surgery for the chest wall tumor and partial resection of the liver were performed. The chest wall tumor, about 3 cm in diameter, was covered with parietal pleura. There was no direct invasions to the ribs or to the intercostal muscles and the tumor was completely ex-tirpated. Pathological examination of therse two tumors showed synchronous metastases of colon cancer.
