内直筋麻痺を生じた原発性蝶形骨洞囊胞例

A case of primary sphenoid sinus cyst with palsy of the medial rectus muscle branch of the oculomotor nerve

Ⅰ　はじめに

　蝶形骨洞囊胞は比較的稀で，その症状も鼻閉や鼻汁といった鼻症状ではなく，視力障害，色覚障害，視野障害，複視，眼瞼下垂，眼痛という眼症状や頭痛が多い。眼症状では視力障害が多くしばしば緊急手術となる。視力障害に眼球運動障害による複視を合併することは多いが，視力障害はなく眼球運動障害のみが生じる例は稀である^1～8）。眼球運動障害を引き起こす神経麻痺として，動眼神経，外転神経，滑車神経の神経障害が考えられ，このうち動眼神経の障害が最も多い^9,10）。しかし，蝶形骨洞囊胞例において動眼神経麻痺のうち内直筋だけが障害された症例の報告はない。今回視力障害を伴わずに内直筋麻痺による複視のみを合併した原発性蝶形骨洞囊胞例を経験したので報告する。

　We reported a very rare case of primary sphenoid sinus mucocele accompanying palsy of the medial rectal muscle branch of the oculomotor nerve without impairment of visual acuity. A 67-year-old male complaining of double vision was referred to our hospital. Ophthalmological examinations demonstrated palsy of the right medial rectal muscle. CT scan showed a shadow in the right sphenoid sinus. The patient was diagnosed as having primary sphenoid sinus mucocele with palsy of the medial rectal muscle. Endoscopic sinus surgery was performed under local anesthesia. The day after surgery,double vision completely disappeared.
