外転神経麻痺を呈した蝶形骨洞囊胞の1例

A case of sphenoid sinus mucocele presenting with abducens nerve palsy

I．はじめに

　蝶形骨洞囊胞は比較的稀な疾患で，種々の眼症状を呈することが知られている¹。眼症状のうち最も多いのが視力障害であり，外転神経麻痺のみを認めるものは稀である。今回，視力障害を伴わない外転神経麻痺を呈した蝶形骨洞囊胞の1例を経験したので，若干の文献的考察を加えて報告する。

It is generally known that sphenoid sinus mucocele is often manifested with ophthamological symptoms. But there have been few reports on sphenoid sinus mucocele presenting with abducens nerve palsy as the only sympton. A 73-year-old women suffered from diplopia due to left abducens nerve palsy. A cystic lesion was found in the left sphenoid sinus by CT and MRI,and it was diagnosed of sphenoid sinus mucocele. The mucocele was drained by endonasal endscopic surgery,thereafter diplopia disappeared soon.
