脳腫瘍を合併したベーチェット症例

A case of Behçet's syndrome with brain tumor

Ⅰ．はじめに

　ベーチェット病は再発性の口腔内アフタ性潰瘍，結節性紅斑様皮疹，ぶどう膜炎，陰部潰瘍を4大主症状とする全身性の難治性疾患である。口腔粘膜病変の存在により，耳鼻咽喉科医がベーチェット病患者を診察する機会は多い。中枢神経障害を合併したベーチェット病（神経ベーチェット病）はベーチェット病の約10％に認められる^1,2）。ベーチェット病に感音難聴やめまいを伴う報告は散見され^3,4），脳腫瘍や内耳障害の鑑別が重要になる。今回，小脳橋角部に腫瘍が認められたが，ベーチェット病があり第8脳神経以外の脳神経学的所見が認められなかったために腫瘍の発見が遅れた症例を経験したので若干の文献的考察を加えて報告する。

　A 54-year-old woman presented with oral aphthous ulcers. She had lost her vision due to Behçet's syndrome 33 years ago. She have not visited the clinic,though she noticed left-sided hearing loss. The oral apthous ulcers were deteriolated,and she was treated in our hospital. After admission,a brain tumor was pointed out by MRI. Her hearing loss was derived from the brain tumor. The rare case of a brain tumor in Behçet's syndrome patient was reported.
