彎曲型咽喉頭直達鏡の応用―下咽頭梨状窩瘻への使用経験

Application of the curved laryngoscope for surgery of the pyriform sinus fistula

Ⅰ．はじめに

　彎曲型咽喉頭直達鏡は佐藤ら^1）により2004年に開発され，近年，下咽頭表在癌に対する内視鏡的咽喉頭手術の際に用いられている（図1）。特に既存の直達喉頭鏡では明視することが困難であった下咽頭輪状後部，後壁に存在する表在癌の治療に有用である。今回われわれは下咽頭の非腫瘍性疾患である下咽頭梨状窩瘻に対して彎曲型咽喉頭直達鏡を使用し，瘻孔の確認に有用であったので，文献的考察を加えて報告する。

　The treatment of pyriform sinus fistula is a complete resection. There is a risk of recurrence if the sinus tract is not completely resected. Some operative techniques are required for the identification of the fistula if the cervical infection has been repeated. For the identification of the fistula we applied a curved laryngoscope which was developed for the endoscopic laryngo-pharyngeal surgery of the superficial hypopharyngeal cancer. The fistula was successfully identified with the aid of the curved laryngoscope dilating the hypopharynx. We discussed the advantages of this endoscopic technique. It could be applied for the benign hypopharyngeal disease.
