副鼻腔炎より波及した硬膜下膿瘍の1例

A case of subdural abscess caused by sinusitis

Ⅰ．はじめに

　副鼻腔炎による頭蓋内合併症は，抗菌薬や画像診断の発達とともに減少しているが，最近でも重篤な経過をたどる症例が報告されており^1），注意を要し，迅速な対応が求められる。今回われわれは副鼻腔炎から波及した硬膜下膿瘍の1例を経験したので報告する。

　A 37-year-old male presented with a subdural abscess secondary to acute frontal and ethmoidal sinusitis. The patient complained of a high fever and left circumorbital swelling. On examination,CT scan and MRI revealed the frontal and ethmoidal sinusitis and subdural abscess.

　Ten days after admission,surgery for his fronto-ethmoidal sinusitis was performed by the transnasal approach. Five days after the operation,the subdural abscess was drained and the cerebrospinal fluid rhinorrhea was sealed with a galea flap by neurosurgeons. No complications occurred following surgery.
