視力障害をきたした副鼻腔嚢胞性疾患の検討

Clinical Study of Paranasal Cysts with Ophthalmological Symptoms

　はじめに

　副鼻腔嚢胞性疾患による視力障害は，急速に進行し予後不良となる症例もあり，手術時期の決定に際しその緊急性の判断に困る場合も多い。そこでわれわれは術前に視力障害の予後を推定することを目的に，術前CT所見と術後の視力の経過を対比させることにより，嚢胞の位置により視力障害の発生頻度，重症度，予後がどのように影響を受けるか検討した。その結果，手術時期の判断の一助となる若干の知見を得たので報告する。

Fifty-three cases of paranasal sinus cysts with ophthalmological symptoms, treated at our department from 1979 to 1991, were radiologicallyexamined to evaluate factors influencing a post-operative prognosis of the visual impairment. When a cyst came in contact with optic nerve either in the optic canal or in its posterior one third in orbit, visual impairment developed rapidly and its progno-sis resulted in poor. Based on this observation, those cases in which cysts invade in the posterior part of orbit seemed to require surgical treatments without delay to prevent permanent visual impairments.
