 A 57-year-old male patient presented with acutely progressive dysphagia accompanied by Forestier's disease. His laryngeal framework was pressed anteriorly with severe tenderness,and the laryngeal elevation was disturbed.

 Laryngoscopic examination revealed that the posterior wall of the hypopharynx showed a swelling,and saliva pooled in the bilateral piriform sinuses. The finding of video-fluorography revealed a stenosis of the hypopharyngeal cavity at the level of the cricopharyngeal muscle. The study of MRI indicated that the cricopharyngeal muscle was swollen,and severe hyperostosis of the anterior longitudial ligament at the level of the 4th-5th cervical vertebra. Then,we performed osteotomy for the hyperostotic lesion of the 4th-5th anterior longitudinal ligament and the cricopharyngeal myotomy. After the surgery,his dysphagia remarkably improved together with clinical findings(videofluorography,laryngoscopy,and symptoms).
