当院における嚥下障害手術例の検討

Surgery to Prevent Aspiration in Our Hospital

I．はじめに

　脳梗塞やその他の中枢性疾患に合併して嚥下障害を発生した患者が，嚥下性肺炎など重篤な状態になるのを防ぐために，神経内科などの他科から嚥下の専門医である耳鼻咽喉科に嚥下改善手術を依頼される場合がある。

　今回われわれは，当院でこれらの中枢性疾患の合併症として発症した嚥下障害に手術加療を施行した症例について検討を加えた。

　Between 1995 and 2001 we performed surgery to prevent aspiration in 14 cases at Kumamoto City Hospital. We made decision on types of surgery to prevent aspiration considering general condition of patients,swallowing function of videoflurography,postoperative rehebilitation and desire of preservation of phonation function. We discussed clinical results in 6 cases of functional surgery of swallowing and 8 cases of diversion of the larynx. The surgery to prevent aspiration was useful in most of cases.
